  • Company ID and affected environment.
  • UTC timestamp and timezone.
  • Endpoint or dashboard page.
  • HTTP status, safe error code/message, and assessment ID when relevant.
  • Expected behavior, actual behavior, and reproducible steps.
  • Business impact and whether the issue is ongoing.

Never include#

Do not send passwords, bearer or refresh tokens, webhook signing secrets, full card data, raw device fingerprints, or unrelated customer PII. Documentation feedback is not an incident-reporting channel.
